HST EXEMPTION
When purchasing adaptive clothing from Easy Fashion you have the option to benefit from the tax exemption by setting up an account and sending us a medical prescription signed by a doctor. This prescription will not have to be renewed and will be kept on file for all future purchases.
PLEASE FOLLOW THESE INSTRUCTIONS:
2 - Download and print the form to be completed with the medical prescription.
3 - Complete the form indicating the resident’s name and nursing care facility (if applicable).
4 - Form to be signed by doctor complete with license number.
5 - Send a scanned copy of the form to info@easyfashion.ca or you can drop into our retail store at 960 St. George Blvd, Suite 141 Moncton NB, E1E 3Y3.
